    PAYMENT OF FEES IS REQUIRED IN FULL,
    AT THE TIME SERVICE IS RENDERED.

    Upon your request we will provide a written estimate of fees. A deposit prior to hospitalization, surgery or extensive treatment is required. There is a finance charge applied to all accounts unpaid after 30 days at a rate of 1.5% per month with a minimum charge of $1.50. There is a $35.00 returned check fee.
